Dear Sir/Madam

I've e-mailed you about this issue before and you sent me a link in your reply which doesn't work.  I completed the form and followed your instrudtions to the letter but it doesn't send the form plus there's no e-mail option as mentioned in your reply.

All I want is to cancel a product that was requested by mistake!
