I hope someone can help me! I've got to fill out a "new patient" form for medical treatment online and it obviously contains personal info. When I hit "submit" I get an immediate warning from AVG about a phishing danger from some seemingly unrelated website (see attached screen grab).

I've tried having other online services examine the site with the form, and all of them (about 70) say they find no problem. I really need to know if it's safe to send my data (which would mean turning off AVG temporarily) or if I need to find a different medical provider?
