Something wrong with the Whitelisting Program Registration?

Yes. I also tried to upload the file to the FTP server. And I need to register to the whitelist program first at the page: Whitelisting Program Registration. And that's the problem I reported at the first place:
When I open this page, Whitelisting Program Registration, I am making an application for our whitelisting program. 
I see the form that requires me to provide an email, a description, reasonable.  But the third field is asking me to submit a file.  I do not know what file to submit here.  I then choose the file that was false detected as virus by AVG.  After that, the submission is successful.  How strange!  I have not got any confirmation email so far.  Really do not know what to do next.  (I think I shall be asked for a password at least, but no).

I experienced the issues on the date I reported the problem, and still today.  I tried both Chrome and IE and both cannot work. 
Please just give me a proper channel for reporting false positive?  (the one at False Positive File Form just cannot work). We are a licensed AVG customer.

An internal error occurred while sending the form.

Actually, the file being uploaded is only 22kb. I zipped it using 7-Zip according to the instruction on another web page.

And the error message is still "The form cannot be sent. An internal error occurred while sending the form. Please try to send the form again later."
