I need the AVG NDA before I can submit our apk for placement on your Whitelist

Our company app is getting some adware detections from our user's AVG software.  We'd like to submit our apk via your online to be checked, but we would like a signed NDA first before proceeding.  As is mentioned on this page you will provide an NDA is desired (AVG Threat Labs - File whitelisting | AVG).

"The digital certificate whitelisting or the account on the AVG Whitelist FTP Server are available upon request. Please contact us using our webform at http://samplesubmit.avg.com/false-detection and choose "Application whitelisting" in the dropdown menu."
