I tried with Chrome, FireFox, IE - all same issue - you can try yourself also and see

After you press the submit button here:
False Positive File Form
you get this:
False Positive File Form

The form cannot be sent

An internal error occurred while sending the form.
Please try to send the form again later.

I am trying to add here but after I add it just delete the commet.
Have you tried yourself to submit fasle positive?
